For Referring Providers

I am a physician, preschool, or other provider working with children and families, how do I refer a child or family to HDS?

With the consent of the child’s parent/legal guardian, you may refer a child or parent to HDS by completing and faxing the HDS Referral Form. Please use the second page and the family’s zip code to identify which HDS office the referral form should be faxed to.

If you are a physician or provider with access to EPIC, you can refer your patients via the Clinician Referral Form in EPIC. This Tip Sheet outlines information about how to access and send this referral via EPIC.

What happens after I refer a family/child to HDS?

A care coordinator will contact the family to complete the intake process and discuss any concerns with the child’s parent/caregiver. The care coordinator will explain all services offered by HDS, and then make referrals or schedule appointments for individual services based on any identified need and parent interest.

You can expect to receive a referral update within 30 days as long as you include your contact information and fax number.
