Physician must complete the ClonoSEQ test requisition form in the Adaptive portal and submit to lab with specimen. Send specimens room temp in the Adaptive shipping kit. Plasma must be shipped frozen within 1 week of collection.
Comments
Insurance authorization is required prior to collection for Outpatients. Ok to send on Inpatients.
