3-5 Slides of Whole Blood or Bone Marrow, 1 Fragment of non-fixed Frozen Tissue in sterile container/tube, 5 FFPE Scrolls (8 microns) in a single block into one tube or 5-10 FFPE Slides (8 microns) unstained without cover slips are also acceptable.

Processing Instructions
HemOnc provider must complete the ClonoSEQ test requisition form in the Adaptive portal and submit to lab with specimen. Send specimens room temp in the Adaptive shipping kit. If same-day shipment is not an option, store specimen refrigerated.
Comments
Insurance authorization is required prior to collection for Outpatients. Ok to send on Inpatients.
