Processing Instructions
HemOnc provider must complete the ClonoSEQ test requisition form in the Adaptive portal and submit to lab with specimen. Send specimens room temp in the Adaptive shipping kit. If same-day shipment is not an option, store specimen refrigerated. Separate plasma within 2 hours of collection and ship frozen.

Insurance authorization is required prior to collection for Outpatients. Ok to send on Inpatients
