Collection Instructions

Collect Monday-Thursday 0900-1300 only, it can be collected on a weekend as long as it can be delivered within 72 Hours of collection and no holidays. Whole blood collected in an ACD Solution A (yellow-top) tube is also acceptable. Submit a completed Platelet EsotericTesting Patient Info form with specimen to lab.

Submit a completed Platelet Esoteric Testing Patient Info form with specimen room temp. Order in Mayolink and send with Mayo Courier Monday-Thursday only.

Insurance authorization is required prior to draw for Outpatients. Ok to send on Inpatients.
