Collection Instructions
Collection device from the reference lab is preferred. Any sterile container is also acceptable. Collection device from the reference lab is located in the cupboard above the processing bench ins the Referral Testing area. No eating or drinking 20 minutes prior to collection. Parent/Patient is required to sign the Calbiotech requisition form and submit to the lab with the sample. Call lab for requisition form. Sample must be received in lab Monday-Thursday before 1100 and Friday before 0900 for sample to be sent the same day. If sent after cut off time, lab will hold sample at stability to send the next business day. Same day results are available if reference lab receives sample Monday-Friday before 1400. Calbiotech hours are Monday-Thursday 0830-1700 and Friday 0830-1500, no weekends or holidays.

Processing Instructions
Complete a Calbiotech requisition form and submit with specimen refrigerated. Parent/Patient is required to sign the form prior to sample being sent. Call a TForce courier for a routine pick up Monday-Thursday before 1100 and Friday before 0900 for sample to be sent the same day. If sent after cut off time, lab will hold sample at stability to send the next business day. Rush pick up only approved for patients going to surgery. Same day results are available if reference lab receives sample Monday-Friday before 1400. Calbiotech hours are Monday-Thursday 0830-1700 and Friday 0830-1500, no weekends or holidays. Make an extra copy of the requisition and give to Ethan or Shiree when sample is ready for pick up.
