1-3 mL Whole Blood collected in an EDTA (lavender-top) tube double initialed (3 mL Whole Blood is required if patient known to have positive Antibody Screen on Type and Screen) ***and*** 1-2 mL First Post-Transfusion Urine collected in a sterile container

Collection Instructions

Must notify Blood Bank via phone with reaction symptoms. Suspected unit must be returned to Blood Bank with the associated IV tubing and solution bags intact. A first post-transfusion urine should be collected and sent to Blood Bank using a patient demographic label.
