Autism Friendly Questionnaire
The Autism Friendly Questionnaire (AFQ) was developed to make health care visits for patients with autism more successful and less stressful. A multidisciplinary team was utilized to help inform the creation of the AFQ including providers from multiple specialties (Emergency Medicine, Surgery, Psychology, Acute Care, Clinical Informatics, Nursing, Child Life, IT), physician and nursing leadership, nursing education, a parent advocate, and autistic self-advocates.
Note, this questionnaire was adapted with permission from Boston Medical Center.
Once completed, the questionnaire is stored in the patient’s electronic medical record and can be viewed and utilized in any department the patient visits within the Rady system of care.
