Accessing Your Child’s Report

After your child’s comprehensive developmental evaluation, the psychologist who completed the evaluation writes an in-depth report. This report documents your child’s strengths and evaluation results. If applicable, the report also lists areas of concern, any applicable diagnosis, recommendations for further support and next steps to access services.

It is important for you to have access to your child’s report, as it may list recommendations for further support and ways you can support your child’s development and behavior. It may also be helpful to provide this report to teachers, therapists or community programs to help determine eligibility for ongoing services.

There are two ways to access your child’s comprehensive developmental evaluation report (please click the link to learn more):

  1. Using MyChart (English and Spanish)
  2. Contacting the Health Information Department at Rady Children’s Hospital (English and Spanish)

*Note* – Only parents or legal guardians are able to access copies of their child’s evaluation reports. Social workers, probation officers, law firms, law enforcement agencies, District Attorney’s offices, etc. should contact the Health Information Department at 858-966-5904.
