Here at Rady Children’s Hospital-San Diego, we are committed to providing a smooth transition between adolescent to adult care. Our goal is to form strong communications between families and teens to help navigate the next step in a young adult’s healthcare journey.
What is Transitional Care?
Transitional Care is the process of providing adolescents as young as 14 years old with the resources and information to find an adult healthcare provider that meet’s their needs. We realize that there are many life changes that come about during the years of becoming an adult, which is why we are committed to aiding adolescents and their families with the tools they need to make a successful transition to adult care.
How We Can Help
Rady’s provides comprehensive care to all teens and young adults until they are ready to transfer to the adult system. Teens and young adults are fully supported at Rady’s for all of their healthcare needs. Rady’s also provides all needed education/support to get teens and young adults ready for the transfer so that they transfer when they are ready.
