The Primary Care Mental Health Integration Program has the following community partnerships:
Children’s Primary Care Medical Group is a pediatric medical group founded in 1995, consisting of 29 locations throughout San Diego and Riverside.
Children’s Physicians Medical Group is a pediatric Independent Practice Association formed in 1998, comprised of CPCMG and affiliate pediatric independent practices.
Vista Hill is a nonprofit social service organization that provides behavioral health services in San Diego County. Programs range from early intervention and preventive work with children before they develop mental health issues, to youth immersed in substance use disorder to long-term special education and training for young people and adults with intellectual and developmental disabilities. Rady Children’s TMH is partnering with Vista Hill on mental health care coordination.
San Diego Center for Children is a nonprofit organization that provides evidence-based therapeutic, educational, foster care and transition age services to children and families struggling with mental, emotional and behavioral disorders. Today, with 8 program sites and community-based services within hundreds of homes and schools across San Diego County, the center empowers over 1,000 people every day. SDCC has two pilot PCHMI sites at the CPCMG locations of Gateway Poway and El Cajon.
UC San Diego School of Medicine established in 1968, is the region’s only medical school. As a top-tier academic medical center, it aims to improve health through innovative research, education and patient care.
National Alliance on Mental Illness – San Diego, is a nationally recognized nonprofit institution with a 40 year history delivering mental health support, education and advocacy for individuals impacted by mental health challenges. In addition, NAMI SD is a creative technology innovator, targeting mental health and wellness for children, youth, families and adults. For example, the organization collaborated with Rady Children’s and the California Interscholastic Federation to introduce depression screening for athletes at pilot high schools. Utilizing contemporary devices and apps enables comprehensive access to health and wellness through numerous digital literacy platforms.
