To help us obtain care for your child, we ask that you fill out the following forms:

1. Please fill out this Inquiry form and email or fax it to our department. One of our International Coordinators will respond within 24 to 48 hours or two business days.

2. Prior to your visit, please fill out the forms below and bring them to your child’s appointment.

• Authorization for Use or Disclosure of Health Information form (English/Español)

During your first visit, please ask the doctor’s office about MyChart at Rady Children’s, which will enable you to view your child’s health information online.
